Agency Led
Agency Led is a CAP/C service that provides a trained caregiver through an agency to support children with complex medical needs in their homes. The caregiver assists with daily routines, medical care, and supervision to ensure the child’s safety and well-being. Caregivers are employed by the agency and receive professional training, oversight, and compensation according to the child’s needs.
Coordinated Caregiving
Coordinated Caregiving is a CAP/C service that provides a live-in caregiver for children with complex medical needs. The caregiver helps with daily tasks, health-related support, and supervision to keep the child safely at home. Caregivers may be family members and receive a daily stipend based on the child’s needs. This service offers consistent, community-based care as an alternative to institutional settings.
Consumer Direction
Consumer Direction allows families in the CAP/C program to manage their own care services by choosing, hiring, and supervising their child’s caregivers. This gives families more flexibility and control over who provides care and how it’s delivered, while still following a care plan developed with their case manager. Caregivers can include trusted individuals or even certain family members, depending on eligibility.
